Primer to Action: Social Determinants of Health

This report was designed as a resource to help better explain the social determinants of health and how they influence chronic diseases. It describes six of the social determinants in detail; income, employment, housing, food security, education and inclusion. Suggestions for implementing changes on these determinants for community organizations and others, are also provided, plus useful links and resources.
